It is something that is the epitome of misaligned or incorrectly placed. For example, "hey Junior, that there wood pile you done made is all catty whompus it done took up too much room and you done laid it all crooked!"
Or "Hey Ernie, that hood on that 1979 Pinto is all catty whompus, you done installed it crooked and it like to never close when I tried to git it ready fer a road test."

Around here, if something's crooked or otherwise out of place, it's catty-wampus. A picture that's not hung right can be catty-wampus.
