Really, poor self control and lack of physical discipline. The muscle that controls ejaculation is the "PC" or "Kegel" muscle, which is also the same muscle that helps women clench their vaginas tighter. It is also the muscle that controls "holding your bladder" and preventing you from urinating.
When properly exercised, this muscle can allow an increase in ejaculation and in addition to that men climax more and it goes farther. The easiest way to exercise it is by practicing "cutting off" your urine flow while going to the bathroom.
However, there is a great book out there for both men and women called "Expanded Orgasm" that details lots of really great techniques for taking the orgasm even further. I reccomend it highly if you feel like impressing your lovers.
