My Saturn has 145,000 mi. on it and I hear a metal on metal clunking noise coming from the front end when i hit a bump. Both front tires are worn on the inside edges. If it is a ball joint problem, what can I expect to spend to repair and does it require a special tool?

Ball joint, worn tie rod sockets, broken spring, strut mounts, so many possibilities, none of which are good driveway projects for a novice to tackle. Get thee hence to an alignment shop for a propler analysis before a catastrophic failure occurs.

The most common part is to wear in your front suspension is the the lower control arm. It has 2 bushings and a ball joint built into it. They aren't cheap at about $200 per side plus installation, but worth it if you like your car enough to keep it. No special tools are required, but it's does take some work to replace them. I would recomment tie rod ends and alignment at the same time. It should make your car drive like almost new.
You may also need struts, but control arms are most likely the problem.

Your car has lower ball joints only. The upper mounts are on top of the struts known as strut bearings.

Yes it does, You should get the whole front suspension checked and also get an alignment, it depends what the problem is. It could cost up to $500. more than what the car is worth. Time for something newer or if it is worth it to you get it fixed.

yes it takes a special tool probably acouple hundred. could even be a strut mount. a good repair shop will be able to figure it out.
