Some lightning occurs between clouds and can involve a relatively small amount of ionisation. Some lightning occurs at high altitute, where the air is thinner, and the pressure wave caused less dramatic. The sound from lightning can be diffracted away by differing layers of air in the atmosphere (i.e. where most of the sound gets reflected upwards).
