Birth certificates are public records, so you need the fee and enough information to locate it, such as name and date, place of birth. In my state, you contract the Bureau of Vital Statistics.

The only thing I needed was my mother's full name (maiden), father's full name, both of their cities of birth, my city of birth, and $35. I did mine online though, and had it mailed right to my house. You can either go to vitalrec.com or just use a search engine and ask birth certificates in (your state).

your mothers name the place and date of when you were born

It depends entirely upon where you were born. The local courthouse is not the correct location in all states.
You can find links to each state at http://www.vitalrec.com/
