You should have cleaned it thoroughly when the injury first happened, dumped a bit of hydrogen peroxide on it, and treated it with an antibiotic ointment.

If he hasn't had a tetanus shot in a few years, one might not be such a bad idea.

Regardless of how it was treated, if it is getting infected you need to go to a doctor and get some antibiotics to clear it up before it grows into a larger problem.

It sounds infected. He should go to a doctor. He most likely will need antibiotics. If this was a regular cut it might be okay to just use neosporin and keep clean, but with a nail there is a risk of tetanus (which can be quite serious), so if he is not up to date on his tetanus booster he should also get a shot.

Im sorry I dont mean this to be rude, but if your that worries to actually have to ask on here then you should be worried enough to go to your Dr's. You might get an infection or tetnas (SP) I would get it checked out especially if you haven't had a tetnus shot in a while

Does it feel hot to the touch? If so, you need to go to the doctor. Is his tetanus shot up to date? If it isn't hot to the touch or if he can't feel his heartbeat in his finger, just keep it clean. Try a 50% hydrogen peroxide-water soak everyday. Swelling would be normal, but if it doesn't start to go down, you really need to get it checked out by a doctor.

If he didn't disinfect the wound following injury he needs to go see a doctor to see if it's become infected.

its infected he really needs to go to the doctor and get it checked out. It could have caused some more internal injuries that he thinks. The doctor can clean it out properly, and give him meds to clear up the infection.
