Boy would my daughter love to have a crack at this question.....smile -think about it - it's a male dominated world. Who owns most restaurants? Who are the pilots? Astronauts? Ministers?
Women have always been the better cooks - but always behind the kitchen door - not at the front of the counter!
When Emeril Lagassi came to the front counter - it "appeared "from that point on - that men were dominating the chef hats - but that's not true - women have been cooking at home for their families for centuries - just not making a 'profession' of cooking.
NOW- the competition is hot! lol Men and Women are at the counter (some husband and wives) cooking together.
My daughter persued Baking and Pastry as a profession and is a Chef on the Norweigian Cruise Line on a Cruise Ship in Hawaii!!! She's loving it -
She has worked with and prepared some special desserts for several 'royalty' and superstars but I'm not at liberty to name names.....(one person tipped her $300!!!! when she worked the Kentucky Derby one year)

It's a good question. (A similar situation exists regarding top hairdressers.) Not sure I know the answer.
One thing that does irritate me a little are answers that insinuate that any career area where men dominate is due to discrimination, but any career where women dominate is due to natural superiority. That's a bit sexist isn't it.
I can only guess at possible causes.
I suppose most kitchens are quite 'macho' places to work - Ramsay in particular seems to convey a militaristic approach where shouting and swearing are common. This perhaps is an atmosphere that doesn't appeal to many women? perhaps more kitchens with more female senior chefs would operate differently and hence attract more females?
The other issue could be that catering is notoriously anti-social and that as more women than men have care responsibilities, it could be an area that more women start and then drop out of, whilst more men are able to continue?

My guess is that the hours and conditions are anti social. Hard to cope if you are a women with kids.Many firms by their definition usually preclude employment if you have child care responsibilities.
To the second part of the question I suppose the answer must be yes somewhere. But the 'stars' and 'royalty' are just customers as we are.
