Definately the ease of operation.
I know you are not supposed to use it while driving, but lets admit, we do make changes, such as zooming in, changing address, while still on the road. Make sure the controls are where you can reach them effectively without stretching too far, dangerous.
As for the program, the 3D kind are nice these days, and you should check to see that it is an update map no older then 1 year from the date.
Voice directions are also very nice.

If the system is car based, it should be easy to update. Lexus uses a DVD based system which is easy to update and is very accurate. If the system is an add-on, you want to make sure the hardware to mount the system is reliable and will hold up. I bought a Magellen 700 series years ago and the suction cup eventually quit working and the unit would fall. I was able to order a better one from them however. I recently bought a new Magellen (The NUVI series) but I first tried the GARMIN units. The Magellen menuing system is much more intuitive and allows you to choose between QUICKEST route and FASTEST route while the Garmin unit simply chooses a route based on how you setup the options in another menu. I also think the Magellen is more accurate, the Garmin would get me to my location but it seemed to choose odd routes while the Magellen selects routes which seem more logical.

Being able to download updated maps or at least getting the new DVDs to update the directions.
Some offer the ability to check with traffic conditions in large metropolitan areas and reroute you accordingly.
If you do a lot of traveling, find one that you can input a destination in a different area of the country without having to put in intermediate destinations because the map doesn't cover that area until you are in that zone. The only trouble with my unit. Other than that, it is a good unit. They are supposed to be coming up with an update, but I haven't seen it, and it has been 2 years.
Some offer hands free ability, and you can speak to the navigation unit and verbally input the address you are looking for.
good luck, I really like mine, just wish they would come out with new software.
