For me it would be an instantaneous evaporation under a Nuclear Blast. That way I can leave a permanent shadow behind if I am far enough from the immediate impact.

A black hole; after entry and upon reaching the event horizon you would separate in two, as you continue you would multiply, breaking up into smaller and smaller parts; you would become trillions of particles. The effort required to leave the hole becomes infinite ~ falling further into the hole is as inevitable as moving forward in time. Regardless, as soon as an object passes within the hole's event horizon, it is lost to the outside world. An observer far from the hole simply sees the hole's mass, charge, and angular momentum change to reflect the addition of the new object's matter. Eventually you emerge out of the black hole into another universe or another part of this universe. =)

I would choose to expire in the middle of the ocean, float gracefully down into the depths being snacked on and reach the bottom as a skeletal structure for some new form of coral.
