I do a lot of customer contact work on the phone. Customers often call, recognize my voice, call me by name then launch into their saga without giving me any clue as to who they are....

And, then some of them are offended when I finally give up and have to ask!

customer should not be asked directly about his name (i was told).

Some people do the same thing to me. It's irritating, especially if they get upset I have to ask, and it's someone that darned well knows I don't know their voice very well. I figure they have such a large ego that everyone should just "know" who they are without being told. Little do they know, I'm psychotic, not psychic. ;)
