what does this usually indicate? What is the best way to fix this

Not a serious operational problem. You need to get your car scanned by an OBD Some auto parts store will let you use a scan tool. The scan will locate the problem, maybe replace a vacuum hose , O2 sensor etc. defective gas cap you may be able to fix it yourself
You can continue to operate vehicle safely, but not as efficiently and you may not be able to get an inspection sticker or pass an emissions test depending upon what state you reside in.

It indicates that you have a problem. It may be serious or not that bad. The only way to know is to have the code read and fix the fault. good luck.

You either opened the gas filler cap on a running engine - ford people do that on a regular basis - or something went south on you. It needs to be scanned to determine what happened.
