Maybe there is a temperature gradient in the tank and the water is cold away from the heater. The heater should be at the bottom of the tank (at the back) so that the heated water rises. If it is at an angle, there will be a cold spot under the high end. Move the thermometer around the tank to identify any cold spots. You may need a higher wattage heater.
