the user!!! theres not really a way to say what is the most common due to the fact we may have the same os gotten from the same disc and mine could act totally different then yours.

if you are talking hardware, anything that has moving parts tends to break before other things. for instance harddrives, fans, etc.
software wise it is a toss up. could be user, program, OS, drivers, etc.
