You can find information you need regarding obtaining workers permit on NY State Department of Labor website. You will have to complete an application form which you can get at your school office.

In order to obtain a full-time employment certificate your parent or guardian must appear in person before the school authorities to give consent.

Ask your local HS guidance counselor. They will have the information you need... or they can get it for you within a couple of hours.

You'll have to get your school to sign off on it... so its the best place to start.
