Go to your local department of motor vehicles and ask for an application.
Follow the directions.

Have you tried calling the Department of Motor Vehicles in Oklahoma. That's the best I can suggest as I don't know OK's codes, live in California

There is a separate manual for motorcycles too. I am a teacher and was able to obtain the book along with the book on tape for my students. I got them from calling the local DMV.
