The most likely thing is a small fissure or tear in the anal ring. This would result in bloody streaks on the surface of the stool. Treatment involves more fluids. If the situation persists, discuss it with your doctor for other ways to soften the stool. Meanwhile, lubricate the anal opening with Vaseline or similar ointment.
Blood mixed in with the stool, not just on the surface, indicates bleeding higher up the alimentary canal. A simple cause could be a colonic polyp, but there are several other causes more difficult to determine. You should seek medical attention in this instance, sooner if there is more than a tablespoonful of blood.

Sounds like he's pushing and the hard poop is causing tears in his bowels. Have you tried any fruit juices or water to help loosen things up? Depending on where you live, the hot weather could make any underlying constipation even worse. Talk to your doctor to see what he/she advises for your child. I wouldn't be too concerned about the blood since its related to the passing of stool. Be sure not to put anything in his bum (like a thermometer) to try to help him pass it - you could tear his colon. If its hard little pebbles, nothing will help him get it out than more hydration, etc. Good luck!
