I was backing up and it sounded like a sledge hammer hit the under side of the truck, then it quit moving and made this horrible whirring sound. I appear to still have 1st and 2nd gear in forward. How bad is this or is it possible it could be fixed.

sounds like something in the transmission broke.. most shops will diagnose it for free, in about 5 minutes... probably has a 4L60E trans.. they will probably give you an estimate of $1500-1800 to rebuild, depending on how many things broke inside it.. might have just blown the reverse band, if all forward gears still work properly...

With an extended cab, long box, there is a steady bearing on the driveshaft. Have it inspected.
