DELIBERATE hurt or cruelty.
I would find it easier to forgive the greatest hurt that is unintentional than the smallest, slightest hurt that is deliberate.
ANYONE can mess up and harm someone else by accident,
To a basically decent person who has accidentally hurt someone, their victim's forgiveness is a welcome respite to their own feelings of guilt. It neutralizes the negative charge on the situation for BOTH the perpetrator AND the victim.
However, the kind of malevolence that it takes to purposefully damage another person cares nothing for forgiveness. To someone acting from deliberately destructive motivations, their victims' forgiveness means nothing more than that they got away with it this time and are free to go on and hurt their next victim without interference. As a human value, it is meaningless to them. It has worth to the sociopathic personality only when it provides a means of escaping the consequences of their hurtful actions, and I am not inclined to aid and abet THAT!

Those that cannot through some mental incapacity of self-choosing distinguish right from wrong.
Oh, mindless litter louts are one thing, but attempting to blow folks up into oblivion is beyond my comprehension.
Despite my Christian upbringing, I cannot truly forgive those that set out to hurt others.
Some idiot last week was driving a stolen uninsured old cat down the local High Street, lost control on the wet road, mounted the kerb & knocked me sideways.
Then drove off at speed.
If I find out through the police who it was, I shall be less than forgiving.
I will thump him on the nose.
Generally, we all make mistakes, so duly forgiven, as I do, having been forgiven for my errors in the past.
Did Jesus forgive His Dad for supposedly allowing him to be crucified. I doubt it.
"Hey, Dad, that was a palaver with the Romans !"
Er, makes you wonder.

I was lied to and cheated on by my ex.
I simply cannot hold grudges with anyone as I am a very understanding person. I hate him though and could never ever forgive this. Even more so because we had a strict no cheating policy. He was my best friend. And because we were going out for nearly 3 years. So there you go. Lying and cheating. Simply because I cannot understand why.

I find it hard to forgive liers, inconsiderate people, thieves, abusers, killers and selfishness.
As these kind of people don't care who they hurt or about the consequences of their doings, aslong as the get what they want.

Unfaithful men/partners.
My husband had an affair and my two significant partners after that had affairs.too. I am therefore finished with men, certainly Cancerians. All three were Cancerians and I am not particularly into that sort of thing, so perhaps it is just coincidence, but I can't help wondering.................!
Joking apart, infidelity of any kind is just unforgiveable.

my own failures; but somehow i am learning to forgive myself because there were stuff i couldn't control anyway. if i knew better, i'd do better.
somehow it's better to 'forgive' (= not to pay too much attention to bad stuff others do) for your own psychological health. that doesn't make you a sucker. you know what's going on and you can confront the offender. but you don't let their garbage bring you down. you / we cant control how others behave.
