I have 2 that come to mind, both played by Christopher Walken. The first would be Vincenzo Coccotti in "True Romance". If you've seen the movie he was pretty deranged when he was tracking down Clarence Worley (Christian Slater) in that movie. The second would be Brad Whitewood, Sr. in "At Close Range". He played a real deranged scumbag in that picture also. But if Christopher Walken is known for anything, it's his ability to play a real heavy.

Although some might not think so, I think its the Joker from Batman. His deranged state is rather proper and its superb, maybe because of Jack Nicholson's usual role of deranged characters. Still, kudos to him.

Malcolm MacDowell as John Rainbird in Firestarter 2. He does all of this really evil stuff and remains affable and upbeat through all of it until the very end. It's as though in his mind, he's benevolent and reasonable.
He also has my alltime favorite bad guy line, "Just because I'm torturing you, it doesn't mean you're not still part of the team."
Crazies who act sane are always the creepiest.

There was a movie, I believe it was called "Elf," that I saw on the monitor at a video store. A full-grown man who believes he is a Christmas elf, and acts like a silly little child. It looked like an incredibly stupid movie.
