I have a game called Medieval Total War 2 and it has Italy split up into three kingdoms.

They are Slicly, Venetians, and Milans... Is this historical?

I mean was there really nations called by these names and did they really fight each other?

Italy was not finally unified into one nation until 1870. Until then it comprised various numbers of states of various sizes. Venice, Pisa, Florence, Pisa, Genoa, Milan, the Vatican (or Papal states) and many others were independent states with their own governments in the Medieval period. Sicily was an independent state called the Kingdom of Sicily (governed by Normans from Normandy for a long time). At various times they were subject, at least theoretically, to the Holy Roman Emperor, but as has been said before this was not holy, Roman or an empire. any control exercised was very loose, if there was any control at all.

I suspect your game has attempted to reduce the confusion by reducing Italy to 3 regions. This is historically inaccurate, but you can understand why they did it.

The unification of the Italian states into one nation took place in the 19th century, lasting many years and many wars. The leaders were Mazzini and Garibaldi. The states fought each other under foreign influence, foreign interests and foreign troops, like the Spanish (from the time of Ferdinand and Isabella to the Holy Roman Empire), later, the French and the Austrians.
The game sounds extremely interesting.

Italy became a united country in 1861. In the middle ages it was a patchwork of city states run by powerful, wealthy families. For instance, many popes came from the Medici family of Florence.

While I think splitting Italy into three nations might have been for ease of gaming, medieval Italy did have many wars.

milan was one of the capitals of the roman empire, after it was split into eastern and western halves.
