First they will measure her height and weight. Then they will look at her eyes ears and throat with that little light dealie. Then they will listen to her heart and lungs and feel her abdomen area for any abnormalities. Depending on the state, they may do some blood work (meaning they will draw a vial of blood from her vein). It doesn't hurt any more than getting a shot would. They may also do a TB test, in which they scratch the surface of the skin with a needle, this hurts even less than a shot does. At fifth grade level, she shouldn't need any vaccinations. Its a very simple virtually pain free process. Tell her not to worry.

first they make them run a mile .. then they make em do push ups and pull ups .. then they take them to a testing facility where they leave them on a former nuclear testing site and see how well their body's can withstand ancient half life radiation.. those who pass will come out unharmed and the failures shall never be heard from again..

Physical? Like medical? Then they just check you out to see if you're in good health. Or for a physical fitness test? I don't know about that one..
