Yeah. Me and the guy got suspended from work for 3 days.(This was 12 years ago, and I was working at Burger King. We were MORTIFIED.)
EDIT:Can't believe I forgot this one(again). When my SO and I were living next door to his twin sister, I thought I would be safe walking nude from our bedroom upstairs to the shower downstairs(which was just across from the base of the stairs, thank God!) Our daughter was watching tv and getting ready to fall asleep. As soon as I get to the base of the stairs, I notice my man and a neighbor(male, of course) standing in the living room. Yeah. They saw me. I yelled at the top of my lungs and ducked into the bathroom.

Yes I was changing out of my costume after performing in a cantata and a woman accidentally walked in the dressing room while I was changing clothes. Oops!

- I was coming out of the shower with just my briefs and my dad saw me
- My doctor saw me just in my briefs
- I was in the locker room stripping and the gym teacher saw me

yeah, I won't go into the em-bare-assing details but a fellow employee went to personnel and accused me of "gay" sexual harassment as a result of a stoopid men's room incident.
