This letter must be sent one full month prior to the end of your rental agreement, unless the lease requires additional time. This letter also indicates your desire to be present for the landlord's inspection for damages to the apartment.

This is a simple letter, just address it to him and state you will not be renewing your lease and that you will be moving out on X date. If you are not sure what the procedures are for turning in the keys, go ahead and ask that in the letter as well.
Also make sure to let him know your new address so that he can send you any remaining deposit that he owes you. (typically it is within 30-90 days depending on the local laws)

Just a basic letter stating the facts. Send it certified mail so you get a reciept that shows you complied with the time requirement.

I found a great web site that it has exactly what you are looking for. If you can't find the exact subject you are looking for, than be creative. Borrow a sentence from one, a paragragh from another, whatever works for you.

You just need to tell him in writing that you'll be leaving and when. Nothing fancy. Don't forget to include the forwarding address for your security deposit.
