Step One: Go to the Court
Step Two: Get the Form

EDIT:

GO TO COURT
GET THE FORM

If you cant figure it out on line, a court clerk will be happy to help you. Dont you think people in the court know more about your small claim than people who have nothing to do with it? Every small claims court has it's own rules. To expect people to know the rules in YOUR court, or to do research for you, is just silly.

You need to go to the court and ask for the relevant form.

Just go right to the court. The clerk can provide you with what you need and can tell you the filing fee.

You don't need a form, although you can use one if you want to. I'd just write a complaint myself. By the way, that's what the document you need is called; Complaint.

you can get this from your local court but remember you have to pay the court fees, which can be expensive
