As explained in the book/movie - Tom Riddle IS Lord Voldemort. The letters in his full name Tom Marvolo Riddle can be re-arranged to make "I am Lord Voldemort".
He didn't want to use his "filthy mud-blood" fathers name and he wanted a name that one day, everyone would fear. So he came up with Lord Voldemort.
In Chamber of Secrets, Tom was a memory - perserved in the diary and he was able to become stronger/more real because Ginny was pouring her thoughts and feelings into the diary and Tom could draw power from that, making her weaker and himself stronger.

Did you finish book six? Because if not I can't give the full answer, lol.
Here's what I can tell you: Tom Riddle, the spirit from the diary, is a sort of fingerprint of Voldemort's former self. As seen in book five, when Dumbledore refers to Voldemort as "Tom" while at the Ministry, Voldemort *is* Tom Riddle. Tom Marvolo Riddle was his full name, and from it Voldemort derives, in Chamber of Secrets, "I am Lord Voldemort." :)

They're the same person, just different names. In the second book, however, the person that's called Tom Riddle is a memory of Voldemort when he was a teenager, stored in his diary as a Horcrux. Voldemort's real name is Tom Marvolo Riddle. When he was older, he came up with the name Lord Voldemort, because he hated his name; the fact that he was named after a mere muggle was a tragedy, in his opinion.

He is Voldemort, or more accurately a piece of his soul. In book two, the Riddle who emerges from the diary rights the name Tom Marvolo Riddle in the air with Harry's wand then reorginizes them to read I am Lord Voldemort.

ok here's the deal about tom and voldemort cuz u see.... tom riddle didn't like his name cuz it's something that makes him similar to other people so he changed his name to be quite unique so from Tom Marvolo Riddle he made an anagram from it and it was I am Lord Voldemort... so tom is voldemort wen he was in hogwarts..... en the tom in the 2nd book was actually a horcrux or a piece of the soul of voldemort placed in his diary
