He is referring to his style. I don't remember the detail, but "macaroni" was a term back then used to describe men who were into fashion and were dandys, basically. If the song were written today, it'd probably say, "Put a feather in his hat and called it metrosexual."
Reading Erica's entry below, she's pretty much correct. I don't know that it was about homosexuality, but it's more the British saying, "These backwater hicks think putting a feather in their hat makes them fashionable and proper." I think the Americans adopted the song to say, "It doesn't bother us what you think."

in the song, it's the feather + cap combo he named macaroni. it was actually a british song they used to make fun of americans, the macaroni was a homosexual club, so the modern equivalent would've been saying "stupid yankee americans, they are so gay"
