He would probably be arrested and taken to prison. It's in the constitution, i think, that a president can only serve 2 terms in the oval office. I don't think the Secret Service would back him, unless in this scenario they had some thing to gain with such a dangerous move. The same for the military, FBI, US Marshalls etc...

It's happened before (in other countries) and it will happen again. Most likely another 9/11 type disaster right before the transition would precipitate such an action and you can bet that the media would get on board with the administration immediately.
I never thought election fraud on a massive scale could happen in the United States but it does. This event could happen, and I'm afraid apathy might allow it.
Even our extended police state cannot prevent violence and riots in large cities. However, the government has proven that it will be very efficient at putting people in jail. Already, the US is well practiced at suspending rights and putting people in jail without charge and without trial. This has already been extended to US citizens.

That's a silly question, but I'll go for the two points. The question you meant to ask is "what happens if Bush does not give up the presidency?" Living in the white house does not make you president. If Bush refused to leave, he'd eventually be evicted.
If Bush refused to give up the presidency, he would simply be ignored. The senate and military and judiciary system would take orders from the new president, not Bush.

Well, it would not be a coup since they would not be overthrowing him -- he would no longer be in power at that point.
Perhaps a team of medical personnel would be brought in to diagnose his problem and respond appropriately. Such response, sadly, would involve the use of a straitjacket.

I doubt he will steal everything in sight like the Clintons did.
He probably won't sell pardon deals to criminals, like the Clintons did.
He hasn't stolen FBI files to blackmail his political enemies, like the Clintons did.
He hasn't been accused of dozens and dozens of scandals, like the Clintons did (needing the liberal hypocrisy machine to give them passes).
