Yes I have. I used to have a friend named Brittney. She was really nice, and for some reason she got mad at me because she thinks I 'stole' one of her poems she wrote. I had no clue she was the one that wrote it. We got in a huge fight that lasted for weeks. But oh well, I'm over her.

Yes, several times. I 'lost' my high school best friend when we were 21. She became very religious and didn't approve of me anymore. I was in college and having fun and partying and she was married with her second child on the way. She just kind of disappeared out of my life.
Another time was when I moved out of state. My then best friend always claimed that he couldn't deal with time zone issues, so he just stopped calling when I was a 3 hour time difference from him. It's stupid, but I know that's how he is because the same thing happened to my sister when she moved away and he no longer called her. It's really weird. I tried calling and leaving voice mails and stuff, but he just never got back in touch. Out of sight, out of mind, I guess. :(

Sadly, yes. One of them moved away and we lost touch, and I'm having issues with one of them right now (Which, if all goes well) will be resolved by the end of the day.

Yes 2 years ago he was killed in a car accident. Before he died he gave me a bear for valentines day and we both agreed that it was ugly, but now I can't go anywhere without it.

Yes. It hurts like hell and there is no going back. You don't want it to happen, but you do something stupid and there's no going back. All you can do is let it hurt like hell and long enough to make the lesson learned never leave your memory. Then you'll never do it again, will you? Will you?

Yes she was an alcoholic and always said she would die the same as her father did, the same age and she did.
