It was Cincinnati's Dan Driessen, in Game 1 of the 1976 WS, Yankees at Reds.

Use of the DH in the Series alternated year-to-year (1976 yes, 1977 no, etc.) until 1986, when the policy was changed to "DH in AL home park only", which continues to today.
