the thumb on the arm you twisted behind my back would be up unless you managed to severly break my arm in the process

I just put my arm round my back for the sake of this question, the experiment concluded: up. However if I kept my hand clenched it would face outwards.

You would see it up in the air, just before the knuckles hit you between the eyes on the forehead. it wouldn't hurt though, not until you woke.
I've been trained in combat and have very fast reflexes.
