This is a sign of violence no matter how you look at it. You need to leave this boyfriend and move on. Anyone that would willingly hurt you physically is trouble. A "titty twister" may have hurt you, and may have hurt you emotionally; but that is a first step in a progression of violence.
Men/boys who hurt women/girls need help. But they need to get help from somewhere else, i.e. not you. And you need to get yourself safe.
You are better than that. Why do I know? Anyone is better than to be treated like that.

Anyone who strikes out in anger has a problem. Just because it was a "really bad titty twister" doesn't make it any less of an assault.
I would break up with him.

Anyone who abuses another is not worth spending any time with. You should run the other direction as fast as you can. If he abuses you and you are just his girlfriend, think how he will treat you as a wife.
