it heats up very quickly from a cold start.--within 1 mile, the heater is very hot. if driven for more 15 miles, the transmission is will not move in forward,only in reverse.and the transmission and engine are very hot, 209~f

209* is not over heating. This temperature is NOT hot. Your cooling fans will not come on until about 220-230*.
Take it in to a Qualified Shop for proper diagnosis. The cheap people will not do well and you will return again and again.
From what you describe, you have a transmission problem.

Thermostat, water pump, radiator hoses collapsing, Radiator needs flushing. Improper mixture of coolant and water. Needs to be a 50-50 mix. Belts or poly belt loose. Transmission low on fluid. Radiator cap gasket or radiator cap bad system can't pressure up.

Oh! here is a little tid bit for you about 94 Dodge Caravans I found out with one I used to have.
Check engine light comes on and off when driving in cross wind or head wind. It is something about where the air intake is located that causes that. You hit a cross wind just right it actually suck air from intake making engine run rich enough for O2 sensors to sense engine running too rich. Then when you hit a head wind just right it rams too much air in and O2 sensors will sense it is running too lean.

Take it to the mechanic before you have more of a problem on your hands.. Check your oil if it looks like water is in it (doesn't look normal) then you have a blown head gasket.. But check with a mechanic a.s.a.p..
