In our town, older homes usually have the bath on the main level and the bedrooms upstairs. Yep, it's an inconvenience but most people adapt.
Typically, what we see happening, is that someone either converts part of a bedroom or upstairs walk in closet into a second bathroom. I wouldn't have a problem buying a house like this is was a bargain & in a part of town I really liked.

You have to keep in mind the trip down stairs whenever you need to use the facility. If a half bath could be installed upstairs,If you bought the house at a bargain price so you can afford to do this,these are the variables. Other wise personally I wouldn't.

In the area I lived in (Ohio) the bedrooms and bathrooms were mostly upstairs. To me, this was inconvienient. Guests had to go upstairs and since I am downstairs more than upstairs, I hated climbing the steps. It's just a personal preference as to what you can/want to live with.

I'm guessing you mean "main level" for the ""downstairs"
I have a house like that. Den, kitchen, Dining Rm, Living Rm, bathroom on main level, and 3 Bedrooms upstairs.
