Voldemort doesn't just hear thoughts without trying, he has to actually focus on them. Just because he can, doesn't mean he does all the time.
He didn't bother at that time because honestly, the Malfoys have been nothing but loyal and he probably assumed that she wanted him dead as much as he did. Not to mention, it was such a high-velocity situation. he was probably overcome with fear and excitement and adrenaline to think properly, let alone wonder if one of his most loyal followers had suddenly turned on him.
But Narcissa didn't lie because she wanted Voldemort to lose, that's wrong. J.K. even says in the book, Narcissa wanted to get back into the castle to look for Draco, and she knew the only way she could get back in is if she went with Voldemort; the Order and the D.A. would have attacked her if she tried to go alone.

Voldemort knew Snape was lying to him about spying on Dumbledore? Snape was skilled at occlumency, which meant that Voldemort never knew the truth about Snape. Dumbledore wanted Harry to learn occlumency so he could close his mind and keep from seeing into Voldemort's mind, which at the same time would keep Voldemort from seeing into his mind. Using legilimency, Voldemort can tell when someone is lieing to him, except if that person is a skilled occlumens. Then he won't be able to see if they are lieing or not. And if the occlumens is particulary skilled, they can let a legilimens like Voldemort see what they want to see while hiding what they don't want to see.
Which means that Narcissa may well be a skilled occlumens and she let Voldemort see her claim that Harry was dead as being true. He wanted to see him dead, and that's what he would have seen in her mind.

Voldemort has the ability to read minds when he tries -- see the discussion of legimency in books 5, 6, and 7. That does not mean that he makes the effort every time that someone is talking to him. Harry looked dead, he sent someone over to check on Harry. He had no reason to suspect that the person he sent over (Narcissa) would be lying to him. As such, he did not take the time or make the effort to try to penetrate her mind.

both ideas above seem to make sense:
1. Voldy didn't bother using Legilimency on Narcissia b/c he wanted it to be true or didn't expect her to lie. One has to use Legilimency, it's not like you hear thoughts of everybody around you.
2. Narcissia could have used Occlumency.
In fact, in HBP Malfoy succesfully uses Occlumency against Snape, and Snape remarks that Narcissia must have been teaching him. So Narcissia must be a good Occlument.

Or Harry is the better wizard, and since his intentions are good that interfered with Voldemort's magic. ...or it's a fantasy and not supposed to make sense.

Well, he looked quite dead, and if your trusted advisor told you he was dead, and you had the most powerful wand, you really wouldn't second guess them, would you?
He doesn't KNOW when people lie to him, because Snape probably did for years because he was secretly working for Dumbledore. He just thunks he does...or did, because Voldemort is dead! Hip hip hooray!

~He trusted her to tell the truth. He did not read her mind because he knew she "knew" that it would be foolish to lie to him. Plus he was terrified. He did not want to take the time to read her mind.
Narcissa lied because she had feared and hated Voldemort ever since he sent Draco on a knowingly suicide mission to kill Dumbledore. She did not want him to win, and she knew that Harry would be able to stop him.

Maybe he wasn't expecting her to lie to him Know what he could do to her family I would expect her to lie.......He could have wanted Harry to be dead so much he ignored his better judgment and believe her.........It seems like it the way hes portrayed.
