He needs proof from the DMV but I don't know how to ask for this form. Can I get this information through the internet or do I have to go in person. Or is there a number that I can call.

for some unknown reason massachusettes does not have in its system that he switched his license. thanks for the info. got a number to call and i'll take it from here.

I would think that a copy of his Florida license would be sufficient. It shows the date of issue. Also, I would attach a notarized affidavit, stating it's a true copy and who made the copy. The copy will show the Florida Driver ID number. With that the Mass. DMV can run any checks they want.

I don't understand - he's required to get a Florida driver's license after moving there, so the Florida license would be proof. "New residents with an out-of-state license must obtain a Florida driver's license within 30 days of establishing a permanent residence in FL. When heading to the DMV, be sure to bring the license from your former state as you will be required to surrender it in order to obtain the new license."

I don't think they can just "switch" drivers licenses from state to state. A person has to go in and apply for the new driver's license, which usually means he has to take a test to get the new DL. There is usually a grace period before the new DL is required in order to continue driving in that state.
