They ask your measurements but I do not believe they actually measure you.

I have been in illinois, nevada, california, and arizona and none of those states measure. They go by whatever you fill out on the application.
I'm not sure what happens when you put down something crazy, like 500 lbs for your weight or 9 feet for your height. But if it's within reason, they don't seem to check (or care).

They don't. I've had an ID in 3 states and they never check. They don't even have a scale in the building promise. Just fill out the form.
Once I was in a crabby mood when I went in to get a new ID and I put down that I was 6'0", blond hair, asian, and 200 pounds. They don't even look. Of coarse I had to get it fixed because clubs thought it was fake.. but it was fun for a while.

Barefoot would be the right answer. That's how tall you are. If you want to wear shoes that makes you look taller, that's your business. It's not a bad thing but you'll still be the same height no matter what.

They usually just take your word for it, at least in the 2 states i have held a drivers license, in Ohio and Missouri. They do give you an eyesight test though.

When I was young and extremely physically active I weighed 109 lbs, now I am only 98-103 but they never ask in my state after the initial form, they just automatically put what you said you weighed when you got your first license.

they ask for that information on the form you fill out. They do not confirm it by putting you on the scale or whipping out the tape measure. You can lie about your weight if you want.

Usually not, but try to be within a reasonable range when you give your height/weight.

No, they just tell you to write it down and they slap it on ur license! It has to be resonable though, in case you get pulled over later.

I don't know where you live, but in here in Ohio, they leave it up to you to fill out all your information to the best of your knowledge. They trust you to be honest about everything. Guess if you have to.
