Currently there are a few others--Mets/Braves has been pretty good the past 6-8 years or so, and the A's/Angels seem to always be duking it out for the AL West every year, although not this year.
Going back 10-20 years, the Yankees/Royals had some great playoff series/rivalries during the late 70s/early 80s. George Brett and Graig Nettles literally hated each other with a passion.
There's also the Dodgers/Yankees who played against each other in the World Series in '77,'78, and '81, plus their rivalry dating back to the Brooklyn days in the 40s and 50s.

most time is Tigers vs. White Sox as both are American League Championships in a row from same division, Division AL Central.

I don't know if they are rivals, but I would guess the Phillies and Pirates since their in the same state. I think the Indians and Tigers don't like each other too much. I'm just guessing on these but those would be my top 2 that I can think of. I can't think of any other possibilities.

i see you like the underdogs. I don't understand why the Cards-Cubs is really a rivalry. they only had like one or two big moments.
