I think that it's a tie between A Nightmare on Elm Street, Freddy vs Jason and Friday the 13th because of the pure guts and gore in them. If you really want some gore, A Nightmare on Elm Street 3 has a disgusting scene where a kid gets his veins pulled out and walked off a building like a puppet!

Nosferatu.
It's the original Dracula. Made by Germans in the early 1900's, it's a silent film but the guy that plays the title character is freaking creepy looking.
