If you have something that is operating smoothly and suddenly something is done that stops it, breaks it or interferes with the smooth running, that is what it means. It goes back to the days of factory work. When someone wanted to stop work for some reason, they tossed something into the machinery to stop it, thus stopping the whole factory.
