put a few grains of rice and a little vinegar in it then swirl around for a bit. it's great for vases too!

I would try Clorox Cleanup in the spray bottles. Spray some inside and let it sit for about half an hour. If possible after sitting, if you can get a baby nipple brush inside, use that to loosen the gunk and then rinse. Make sure you wear gloves when using the Clorox Cleanup and the bathroom well ventilated. I hope that works for you. If all else fails you can always use a short but pretty flower vase for you toothbrushes. It will be easier to clean.

First of all, throw away your toothbrush holder, and get a small glass, like a shot glass, and put mouthwash in it. Put the toothbrush in it after you brush your teeth. Its healthier that way, and you can really see results, when you wash out the glass every few days - there are a lot of germs on that toothbrush that gets washed out.....you wouldnt have to clean any holders.

Bleach, Clorox Cleanup or hydrogen peroxide. No need to dilute any of them. Just pour or spray enough in to cover the bottom, leave to soak for 20 minutes, then rinse out with hot water. If you use bleach or Clorox Cleanup, be sure to open the windows. Bleach fumes are harmful to your lungs.

Ditto "Hasbeen" - if things won't go in a dishwasher or washing machine, I really don't want to know.
If you don't have a dishwasher, do you have a friend who does? You will be amazed at how clean the toothbrush holder will come out.
Or, "Lakeland" sell something called "Magic Balls" which are sold for cleaning things like glass decanters where you can't reach inside to clean (they do work) - see their virtual catalogue on their web site.
Or - you could change it for one which is easy to clean and donate your old one to a charity shop.

If you don't have a dishwasher(which i don't)Try denture cleaning powder. Leave overnight then stick a bottle brush or similar down the holes and rise in soapy water.

Hot water and bleach. Let it soak awhile. Use 1/2 bleach, 1/2 water. If it's really dirty, use 100% bleach and rinse with hot water.
