The Russian Tsar is supposed having used this term first describing the (decline of the) Ottoman Empire (Turkey).
In modern times this term is often used for a european country which is at that time facing economic problems: UK, Spain, Germany, Italy, etc had been labelled by this attribute, and more recently Russia, Portugal and France.

I believe the answer is (or, rather, was) Germany; the country got that unflattering description due to its economy's poor performance. Now, due to its economic revival in recent years, it is more often called the engine of European economic growth or something to that effect.

when that term was used turkey was occuping the balkans.
turkey is no longer in europe so is no longer the sick man of europe, but it's still a sick man ( or woman )
