He wasn't the first to do it, he won't be the last. It was just his own name because "Crippler" was his nick name. The version he used actually was developed by Dean Malenko. There are other versions of the cross-face that have been used as well.

I doubt it. Because Vince is all about making the company look Good so he probably doesn't want anyone to do it because he thinks that the media will think it is a tribute to Benoit.
