You say add water... do you mean like topping off for evaporation? If it's only about 10% of the total pond volume you can. Adding that much without dechlorinator won't harm anything, but I wouldn't go over about 10% without treating it.

You can, and I have, without adverse effects, but like Magicman said, only in small amounts. When I replace what has been lost to evaporation, I am only replacing about 300 to 350 gallons out of a 3,000 gallon pond (about 4" at a time, is all). If I am going to do a bigger water change than that, I use a dechlorinator when I refill.

Only if you have non-chlorinated well water. Any city water in the U.S. will have chlorine or chloramines in it. This must be removed, or you won't have koi for long.
