we moved and i can't find it, do i have to report the card lost, which office, if any, do i go to replace the card. thank you.

I would talk to the Dr about whether or not to report it or not..but your previous Dr should have all that on file and everything should be documented in the medical records. You may have to write a letter to the previous Dr to see if they will release those medical records to the new Dr anyway.

contact the dr that gave her her last set of shots, they will have either a copy in the chart or be able to make out a new card from their records. You may have to pay for a copy, but tis usually $10 or so.

There should be a childhood registry that gets sent information every time your child has a vaccination that information goes to them so they know if your child is up to date with their immunisations if you find their number you can call them and get them to send you a copy of your daughters immunisations. Or you could go o your whoever gave your child their immunisations and ask them for a new copy of the immunisation record or ask your Dr or early childhood nurse how you can get another copy of the records.

Call the pediatricians office. They'll have all the proof of your vaccines and can give you another card.

Your daughters pediatrician can make you a copy also you can check with your state health department.

your kids doctor should have extra replacement cards. and they can fill it out for you

only you and your doctor should have copies..unless your doctor has a copy there is no way of finding out..
