Well, I fully support his decision to invade Afghanistan, and his role in making the Do Not Call Registry to stop phone solicitors a reality. Other than those two things, just about everything he's done as president has been an unmitigated disaster!
Although Planksheer, the answerer above me, mentioned some DARN good points, if I had to choose the absolute worst of the worst, I think it would be a tie between:
1) His decision to piss away billions of taxpayer dollars in order to invade and occupy Iraq, a country that had NO WMDs, NO ties to Osama, and NO part in the 9/11 attack. Especially since the job in Afghanistan was (and still is) nowhere near complete!
2) His disgraceful environmental policies are nothing short of a complete and total @$$rape of Mother Nature. He's kissed up to the lobbyists of greedy, corporate polluters, and managed to undo years of environmental progress in this country.
Sadly, the BushCo administration is a total train wreck.

I would argue that the advesarial relationship that Bush has fostered with members of the opposition is the worst. Yes, there will always be a lot of animosity between the parties, but Bush has consistantly refused to compromise on most issues, which really fed into the anger of the Democrats. Reagan did a lot the Democrats hated, but he was also willing to compromise and meet Dems like Speaker Tip O'Neil halfway on a number of issues. Bush and his close cadre will either make no concessions, or only token concessions that he knows will be rejected.

I would say allowing torture, suspending habeas corpus, politicizing the Federal government, ( not just the DOJ) , his bogus war in Iraq...and stealing the 2000 election. His attempt to create a one man, one rule state tops everything.

Invading Iraq when the job in Afghanistan wasn't done. Now we have a mess in Iraq that we created, and a mess in the border area of Afghanistan and Pakistan that we let get worse and worse while we were wasting blood and effort and money in Iraq.

There were soooo many things that were really bad it is hard to determine the absolute worst. Perhaps the decision to go to war with Iraq was the worst if I absolutely HAVE to pick only one.

-The war has eaten up over $500,000,000,000 taxpayer dollars.
-Lost over 3,000 lives of soldiers (untold number of contractors and Iraqi citizens).
-Strengthened Al-Qaida's recruiting tools (for personnel and finances)
-No end in sight
-Little to no benefit to the US taxpayer overall. (eg: was it worth $500,000,000,000 just to hang Saddam?)
-Reduced America's standing around the world.
