How do you put real estate into your last will when you are living with someone and have grown children?

The house is in my name and my girlfriends.

You put it exactly how you want it to be when you pass. Mine is going to be totally given the the ASPCA if my hubby dies before me. It is all spelled out in our wills. It specifically says that nothing will be left to his children (I do not have any) his kids are grown.
