When a male coughs his testicles tend to raise up slightly, the doctor is checking to see how far up they go, if they rise to high this could be a sign of a hernia.

It's not a testicle exam. It's a hernia exam. By pressing up and having you cough they can see any weakness it the wall.

increased abdominal pressure, the examiner's finger is up the inguinal canal, if the guy has a hernia, it might get pushed down the inguinal canal and touch the examiners fingertip when the patient coughs, if that happens, you could suspect the patient has a hernia

he is basically feeling a muscle above the testicles, and when you cough it will move (if there is no hernia) if there is a hernia the doctor won't feel anything move there because the muscle is torn
