The big concern about smoking is lung damage, but when you smoke that smoke and the "tar" in the cigarette smoke get into your mouth. Most smokers get lung cancer or cancer of the larynx, but some smokers get cancer in their tongues or the other soft tissues in the mouth. That smoke and tar can also cause peridontitis, which is an advanced gum disease that is very bad for your teeth -- advanced cases of peridontitis result in destruction of the bones that hold your teeth in place, so you end up losing your teeth. Not good.

I don't know the medical answer to this, but I would 'think' that if someone can get cancer from 'secondhand' smoke, that you also could just smoking but not inhaling. My grandmother smoked for years and did not inhale. She said that it was always just a 'fad' and they did it to be stylish way back then. She did get cancer (thankfully they were able to get rid of it). She has since quit smoking period.
