I find it very unusual that I could not find a word for it. There are phobias for just about everything. Fear of elevators is fairly common, so one would think there would be a name for it, but after search of various web sites, alas, I couldn't find one, and I have a mild form of it.
Later: I emailed my therapist and she told me that there is no name for it.

People just say elevator phobia if its not related to another fear.

There is no one definition for elevartors, though most with this anxiety suffer from one or both of these:
claustrophobia - fear of small spaces
acrophobia - fear of heights

Well, an elevator is a very small space, and if you are afraid of small spaces that is called claustrophobia.
