The outcome depends on what system you are talking about, but they all work basically the same.
A 3.0 GPA is less common. A 4.0 is the one most educational systems use.
If you are talking about percentile GPA then it is based on the total score of all exams divided by the number of exams. This give you your 'percent'. Ex. 5 tests taken with various scores; 83%, 95%, 93%, 89%, 98%, would have a GPA percentile of 91.6%.
From this is derived the point GPA style. I don't have a chart right in front of me, but they basically have a scaled chart that a percentile falls under or they have a formula that determines a GPA with your percentile.
Don't be discouraged if you are not 3.0 or 4.0 -that is perfect (or very close to it)! My graduating (college prep.) percentile was near 97% and my GPA was something like 3.78 -as best as I can remember, its been a while.

Grade point Average works to to give an overall view of how you have generally done in school.
Each grade has a certain amount of points.
A = 4
B = 3
C = 2
D = 1
F = 0
All your grades are added together and then averaged.
For instance you get 2 As, 3 Bs and one C
4+4+3+3+3+2= 19 divided by 6 the number of grades equals a gpa of 3.16

In college, it is as follows
0% to 69% = 0 (F)
70% - 79% = 2 (D)
80%-89% = 3 (B)
90% and higher = 4 (A)
